NO GAP Check-up and Clean — Terms

• Gap Free promotion only applies on weekdays at certain locations.
• Check-up and clean and x-rays may have an out-of-pocket gap payment, check individual locations for more information.
• General Check-up, Scale and Clean, Intraoral x-rays, OPG's, Lateral Cephs are gap free, provided that the health fund pays money towards the treatment (i.e. 011 012 114 111 121 072 022 037 item numbers).
• It is the responsibility of the patient to check if your health cover still has dental funds.
• The Scale and Clean does not include a second visit for further treatment, the use of anaesthesia, deep cleans, periodontal charting.
• This offer is available once in a six month period.
• Health Fund cards must be present on day of treatment or full price will have to be paid and patient will have to claim for themselves.

Patient Referral Program — Terms and Conditions (West Ryde)

1. Eligibility — The Program is open to existing patients of My Local Dentists West Ryde who are 18 years of age or older. Referrals must be new patients who have never visited My Local Dentists West Ryde before.
2. How It Works — Existing patients ("Referrers") can refer friends or family by having them use a referral card available at reception. The referred person ("Referee") must schedule and attend a new patient appointment for the referral to qualify.
3. Referral Rewards — Referred patients will receive 10% off their dental treatment, exclusions apply, valid for one treatment only. Rewards cannot be exchanged for cash or transferred to another person.
4. Limitations and Exclusions — Referrals of immediate family members living in the same household may not qualify. Referrals must be made before the new patient's first appointment is booked. 10% off discount applies to out-of-pocket costs and cannot be combined with insurance-covered services or other promotions.
5. Fraud and Misuse — My Local Dentists West Ryde reserves the right to disqualify any participant who attempts to game or abuse the referral program. Suspected fraud or misuse may result in termination from the Program and cancellation of any pending rewards.
6. Program Changes — My Local Dentists West Ryde reserves the right to cancel or modify the Referral Program or these Terms at any time, without prior notice. Any changes will be effective immediately upon posting on our website or at the clinic.
7. Legal Disclaimer — The Referral Program complies with applicable state and federal healthcare marketing laws. No part of this Program is intended to induce or reward patient referrals where prohibited by law.

Leichhardt New Patient Consult and Digital Scan Offer

• This offer is available exclusively at the My Local Dentists Leichhardt clinic for new patients only.
• Offer Inclusions: One new-patient dental consultation and one digital scan for cosmetic, orthodontic, or implant dentistry. Total value: $190.
• Eligibility: Available to new patients only. Patients must be 18 years or older, or accompanied by a parent/guardian.
• Booking Requirements: Appointments must be booked in advance.
• Exclusions: General dental treatment (e.g., cleans, fillings, X-rays) is not included. The offer cannot be used for emergency appointments.
• Non-Transferable: This offer is non-transferable, cannot be redeemed for cash, and cannot be combined with any other promotion or discount.
• One Per Person: Limit of one complimentary consultation and scan per patient.
• Validity Period: Offer is available for a limited time only. The clinic reserves the right to end or modify the promotion at any time.
• Results & Treatment Plans: Any recommended treatment following the consult and scan will incur additional costs. Patients will be provided with a treatment plan and fees before proceeding.
• Cancellations: The clinic may request at least 24 hours' notice for appointment cancellations or rescheduling.
• Clinic Rights: The clinic reserves the right to refuse service where appropriate or if the offer conditions are not met.
• The patient must mention this offer at the time of booking to be eligible.

Leichhardt Invisalign Promotion — Terms and Conditions

1. Promotion Overview — This Invisalign promotion is offered by My Local Dentists Leichhardt and includes a free consultation with a 3D scan, free Invisalign tooth whitening and a $500 discount on Invisalign treatment plans that meet eligibility criteria.
2. Eligibility — Open to new and existing patients, aged 8 years and over. Participants must be clinically assessed as suitable for Invisalign treatment during their consultation. A minimum treatment value of $4,000 applies to qualify for the discount.
3. Consultation Details — The complimentary consultation includes an oral assessment and 3D intraoral scan. The consultation does not guarantee eligibility for Invisalign treatment.
4. Discount Application — The $500 discount is applied to the total cost of an eligible Invisalign treatment plan. It will be reflected in the final treatment invoice once treatment has commenced. The offer is not redeemable for cash or transferable.
5. Limitations — Cannot be used in conjunction with any other discount or offer. One promotional discount per patient only. Treatment must be performed at My Local Dentists Leichhardt.
6. Changes and Cancellations — My Local Dentists Leichhardt reserves the right to modify or cancel this promotion at any time without prior notice.
